Requirements for Instructors and Students

Please complete the following videos and learning modules to fulfill your orientation requirement PRIOR to your first day in the department and before patient contact.

  1. We Promise Video
  2. Learning Module 1 (Listed under Step 2: Complete Orientation Training)
  3. Learning Module 2 (Listed under Step 2: Complete Orientation Training)
  4. Affinity Student Orientation Module

    5. (If the slide shows for items 2, 3, or 4 do not automatically start, press F5 on your keyboard to start the presentation)
  5. Corporate Intregrity Video
  6. Values Based Behaviors Expectations

The following forms must be completed and returned to Affinity Learning Center at Mercy Medical Center:

  1. Confidentiality Agreement. Located in Learning Module 2
  2. HIPAA and Blood Borne Pathogens Confirmation of Completion. Located in Learning Module 2
  3. Attachment B – Student/Instructor Acknowledgment

    4. This must be completed the 1st day in the department/facility.
  4. Attachment A - Student Orientation Checklist
