The Community Endowment Fund has existed since the early 1980s. The St. Elizabeth Hospital Foundation grants 5 percent of this fund’s annual earnings to area non-profit organizations serving the poor and/or focusing on community health initiatives.
Criteria and Application for Funding
Requests for funding must meet one or more of the following guidelines.
Funding Guidelines
Funding is based on the following criteria.
- Organizations may not have received funding from St. Elizabeth Hospital Foundation for the past three consecutive years (i.e., an organization that received funding for 2007, 2008 and 2009 is not eligible to apply in 2010). The agency may re-apply after skipping one year.
- Monies must be used to support organizations within Affinity Health System’s Northern Region. This area is defined as Appleton and all communities north of Neenah (including Neenah), extending west to New London and Clintonville, and east to Kiel and Chilton.
- The Endowment Fund will address the needs of the poor, disadvantaged and medically underserved.
- Capital projects will be considered, but emphasis will be placed on programming and operating support.
- Multi-year pledges will not be considered.
- There is no specific range for dollar amounts approved per grant; however, grants typically range from $1,000-$10,000.
- Notwithstanding any term or provision to the contrary contained in the preceding sections, no grants shall be made to any local, state or federal governmental unit that is tax supported, nor any unit of government or any non-governmental organization that seeks to lessen the cost burden of government (as set forth in IRC reg 1-501(c)(3)- (1) (d) (2)) nor shall any grant be made to any “action organization” as defined in IRC reg 501(c)(3)- 1 (e)(c)(i).
It should be noted that projects and services that help the poor are given priority for funding. This is in keeping with the St. Elizabeth Hospital Foundation mission to "live out the healing ministry of Christ with a special emphasis on those in need, by generating, stewarding and distributing funds to enhance the quality of Affinity Health System’s services and programs and other community health care initiatives."
Grant Application Procedure
1) Grant applications must be received by Nov. 1 of each year.
2) Complete the electronic application form below. In addition to completing the application form below, please forward a copy of the IRS determination of your organization’s tax exempt status, the articles and bylaws, a list of your current board of directors, and a current operating budget via e-mail to Liz Rice Janzen at ejanzen@affinityhealth.org.
3) You will be contacted if the grant committee wants to meet with you. Final decisions will be communicated to you via e-mail by Dec. 31. Please do not contact the foundation office regarding the status of your application.
Reporting Requirements:
Organizations receiving funds from St. Elizabeth Hospital Foundation are required to provide a year-end report (due Oct. 1) detailing the progress that has been made toward the objectives outlined in the approval proposal. Organizations that do not comply with reporting requirements may not be eligible for future funding from St. Elizabeth Hospital Foundation.
