Job Shadow Information and Instructions
Information and step-by-step instructions for students and faculty to request a job shadowing experience. NOTE: Those students requiring more than eight hours must submit a letter from their school verifying the requirement.
Please note: Job shadowing placement is determined by patient/resident volume, availability of mentors and the involvement of clinical students in the department. Please feel free to contact the Affinity Learning Center at (920) 223-0288 if you have any questions.
The process for requesting a job shadowing experience:
1. All students who are interested in a job shadowing placement are to complete a "Job Shadow Request" and review the "Information Sheet." The students are to return their completed form to their school representative and have the school complete the "Signature Form." Each school representative will then pre-screen the registration information for completion of the following:
- All pertinent information has been provided.
- The student’s objectives fit the request for the "Job Shadowing" experience.
2. Job Shadowing experience is limited to eight hours. Those students requiring more than eight hours must submit a letter from their school verifying the requirement.
3. After the pre-screening has been done, please mail the signature form, health form and the copies of immunization records and/or lab results to:

** The request form, health form and copies of immunization records and/or lab results MUST be received by the Affinity Learning Center department within two weeks prior to the actual start date. Request for job shadowing placement that fails in doing so may be denied.
