Wisconsin State DNR applicants must pay the initial $35 enrollment fee; annual fees are waived.  Wisconsin State DNR applicants must include the State of Wisconsin Do Not Resuscitate form with MedicAlert® application.  Wisconsin State MedicAlert members who elect to have the state authorized DNR emblem must choose the bracelet styled emblem.
