When you need rehabilitation after an injury or illness, you may experience physical as well as emotional challenges.

Affinity’s Rehabilitation Services helps patients in Northeast Wisconsin achieve their maximum level of function and independence. We have a range of services to address your physical, emotional and spiritual needs at several convenient locations in the Fox Cities. Our goal is to improve your quality of life as much as possible.
Rehabilitation Membership Program
Once you’ve met your rehabilitation goals and are discharged from Affinity’s rehabilitation program, you are welcome to keep using our facility and equipment. Many former patients like the guidance of working with a trained staff, so we are happy to accommodate them (small monthly fee). We’ll help you set up an individualized program. No physician referral is necessary.
